Current Referral System

Southwark

Home & Dry staff attend a Southwark accommodation group with nurses, support staff, housing workers, hospital bed managers.

This is where individual clients are prioritised to be discharged from hospital back to habitable accommodation.

Clearly, with the cost of an average weekly hospital stay nearing £3000, southwark save a great deal of money with clients being discharged sooner, and Home & Dry have a clearly defined referral process with a general realistic appreciation of the work involved, how long tasks take and which client is a priority.

An added extra is the unquantifiable saving from the preventative nature of home and dry's work in sustaining people in their homes for longer periods of time.

Lambeth

There is no one identifiable referral source and no agreement as to the numbers of clients that home and dry can realistically provide a service to.

Referrals come from many community teams and there is a consistent waiting list. Inevitably it is Home & Dry as a team who prioritises which clients/jobs we can do at which time.

There is a considerably smaller amount of hospital referrals via Lambeth compared with Southwark
